FOR GRADUATES OF THE PIEDMONT M.A.T. PROGRAM
WHO HAVE NOT RECEIVED ANY TYPE OF
TEACHING CERTIFICATE

In Summary, this is what you will be doing:

  1. Set up a MyPSC account with the PSC.

  2. Fill out the Georgia Application for Certification (see instructions and form below) .

  3. If you passed any of the Praxis tests - attach a copy of your Praxis I and/or Praxis II score report.
    If you passed any of the GACE tests - you do not need to attach a copy of your score report. The PSC already has these on file.
    If you EXEMPTED the Basic Skills test - attach a document copy showing the valid scores used to exempt the Basic Skills test, such as SAT, ACT or GRE scores. These do not need to be official documents.

  4. Attach official transcripts from every school that you attended before Piedmont. We will not process your application until all transcripts have been received. (Note: these transcripts may not be released from your Piedmont Admissions File. New ones must be obtained).

  5. Fax, mail or drop off your application and documents to the Certification Officer at Piedmont.

Step 1: Set up a MyPSC account via the PSC website:

A MyPSC account MUST be created by anyone who has taken the GACE. To view or print your certificate, or to read correspondence from the PSC, you must log in to you’re MyPSC account. The PSC will notify you via email when new correspondence has been posted, including when your certificate has been issued. Everyone will need to take a few minutes to create his or her account. If you have not yet taken a GACE test or had contact with the PSC you will need to set up your account after your certificate application has been submitted and the PSC has notified you via email.

You must have your PSC ID number to create and log in to your account. Your six-digit number is printed on correspondence from PSC as well as your GACE test results. If you do not know your six digit number then go to the PSC home page, click on the blue button “Check Certification Status”, and search for your name. Your number will show at the top of the page with your information.

To set up your account go to the PSC homepage (www.gapsc.com) and click the link that says, "Register my PSC" in the top right corner.


Step 2: Print the Applicaton form. Please follow the instructions below to complete:

Step 3: Complete the Application:

Item 1:
Use ALL CAPS in the top portion and BLACK INK throughout the form. Be neat! These forms are scanned and will be rejected if not complete or clearly written.

List your name as you want it to appear on your certificate. The address you list will be the address that your teaching certificate is mailed to. Put down a stable address where you can get your mail, even if it is not where you live now.

Item 2:
If you are already employed by a school district put down the name of your school district (not your school name). Employment does not include substitute teaching. If not employed in a school district write “N/A”.

Item 3:

A. Bubble in the fourth item down: “Clear Renewable Certificate ”.

B. List your teaching field on the blank line at the bottom of section 3. Include your concentration if you are getting Middle Grades certification (i.e. Middle Grades: Language Arts & Social Science) or Secondary.

C. Leave the "Amount of Fee Submitted" blank. Your first certificate is free.

Academic Record:
List each college you have attended, even if you just took one course there. Beside the college name put the approximate dates you attended (i.e. Jan 2000—May 2001) and then the degree you received, or N/A if you did not received a degree. Beside Piedmont College your degree will be “MAT”.

On the row for Piedmont put a √ in the column for “Transcript Attached”. We will get and attached your official transcript from Piedmont. You must obtain and include an official transcript for each of those colleges. We cannot use transcripts previously submitted to Piedmont with your application to the college. Please submit these wtih your Application in their sealed envelops.

Certification Record:
Skip this middle section. It is for teachers who have been certified in other states.

Assessment Record:
List on the first row Praxis I or GACE Basic Skills tests. If your test is a Praxis test you will need to include a copy of your score report(s) when you submit your application to Piedmont for processing. Check the box that says "Copy Attached". If your tests were GACE tests then they should be on file already with the PSC, unless you did not request your scores to be reported to PSC at the time you took them. Write the words "On File" under the column "Copy Attached". On the next row list your Praxis II or GACE II Content Test area, include the same information as for the Basic Skills information.

If you have alternative test scores for Basic Skills, such as SAT's or ACT's, you must provide a copy of these test scores when you submit your Application to Piedmont. This can be a copy of a high school transcript, a test agency report, or other official document copy. These must be included with your application to PSC or they will reject your application. Write the word "EXEMPT" in the box under "Date Passed".

Personal Affirmation:
What you indicate on this form must match any background check performed. You must be honest or PSC will NOT issue your certificate. Follow the instructions on the form carefully. Bubble in the appropriate answers. Don't forget to sign the form. The signature date cannot be more than 80 days prior to your graduation date.

Please call either the School of Education Dean or the Education Certification Officer at Piedmont if you have questions or concerns about your Personal Affirmation. We would be happy to discuss with you privately any concerns you have. If you check "Yes" for one of the questions then you can expect that your application will take much longer to be processed by PSC.

Sign and date the bottom of the form. Be sure you do not sign the form more than 80 days prior to your graduation date.

Please Note: If you are already teaching with a non-renewable certificate your employer may want to process your application themselves. In this case you will need to request your official Piedmont transcript yourself AND notify me that you require a Program Recommendation. I submit the Recommendation form electronically.
