Course Completion Form If you are applying to the LPN-BSN Nursing Program or Pre-Licensure Nursing Program please complete the form below to verify that you will complete your general education course requirements before beginning your Nursing Program.

LPN-BSN: Complete or plan to complete all general education courses with a minimum of a “C” before fall 2013. If the initial grade is a “C” or higher, repeat grades will not be counted. Must have a minimum GPA of 2.5.

Pre-Licensure: Complete or plan to complete all general education courses with a minimum of a “C” before summer of 2013 for Demorest and fall of 2013 for Athens. If the initial grade is a “C” or higher, repeat grades will not be counted. Must have a minimum GPA of 3.0.
