Pre-Licensure Application Requirements

IMPORTANT: You must complete ALL the steps below to be considered for admission to the School fo Nursing & Health Sciences.

1) Apply to Piedmont College- you must be accepted prior to December 1st

2) Complete or plan to complete all general education courses with a minimum of a “C” before fall 2013. If the initial grade is a “C” or higher, repeat grades will not be counted. Must have a minimum GPA of 3.0.

• Refer to the transfer chart for equivalent courses at another institution.

• Fill out the online Course Completion Form

OR

• Download, print, and complete the Course Completion Form (Return completed form to the School of Nursing and Health Sciences)

3) Send Transcripts from all colleges previously attended to the PC Admissions Office

4) Complete the TEAS V exam - you can take it no more than 2 times per academic year and must wait 30 days between each attempt - results must be from within the academic year of application.

• TEAS V scores need to be sent to the School of Nursing and Health Sciences

5) Completed PC School of Nursing and Health Sciences Application.

• Any applications that are not completely filled out will not be considered

If you qualify for the program, we will contact you to schedule an interview.
All interviews will take place on Jan. 30th, Jan. 31st and Feb. 1st, 2013.

It is your responsibility to make sure that Piedmont College and the School of Nursing and Health Sciences have all of the necessary documents prior to the January 15th application deadline in order for your application to be considered.
