2. Final, Official transcripts from all colleges and universities attended. These must be submitted in sealed envelopes sent from the registrar’s office. (Piedmont College transcripts are on file.)
3. Three professional references using our online professional reference system.
4. Personal Affirmation Form
5. Cleared Certified Background Check. Complete the background check request along with fee at certifiedbackground.com.
Proof of Current Background Check. If you are a full-time employee of a school district, you may complete this form instead of the background check.
